Record Subpoena Preparation

If you'd like to eliminate some headache from your job, allow 1st Nationwide to prepare and serve your subpoenas for records. Do you forget what the timeframes are for serving the consumer, notifying opposing counsel etc? Don't worry about those details; let 1st Nationwide handle all that for you.

If you need to obtain a patients medical records, employment records, financial records we can help. If it's business records you need subpoenaed and copied we can help with that too! Using our state of the art software, we can prepare state, federal and worker's compensation subpoenas. We also prepare and serve all the necessary supporting documentation (NTC's, Declarations for COR, opposing counsel, etc). We serve opposing counsel, the notice to consumer and the witness, all following the guidelines specified by the California Code of Civil Procedure. After the subpoena has been served a complete set of the documents are returned to your office along with the proof(s) of service for each witness. We then work with the witness to get the documents copied on the appropriate production date.

We can provide these documents to you in a variety of formats. You can choose to receive them on CD in either TIFF or PDF format or we can provide them in hard copy (bate stamped if necessary). We can also scan the documents using a program that automatically allows you to load the file into Summation.

If you have an approved credit application on file with us we will be happy to advance and bill for any allowable fees requested by the witness.

Medical Authorizations

If your client has signed an authorization for release of their medical records and you need a company to serve the authorization and then photocopy the records we can help. Many of the local hospitals now have their own authorization forms that meet HIPAA requirements. We have these forms on hand at our office and will be happy to assist you with coordinating the copying of the charts. We can also provide you with a basic HIPAA compliant form if necessary.
